Office Policies

Payment in full for amounts not covered by insurance are required at time of service for all consultations/office visits. No exceptions.
Bring current medicines, insurance card(s) and drivers license to all visits.
Cancellations within less than one business day and or "no shows" in the office are subject to a $50 charge.
Cancellations of procedures within less than two business days are subject to a $150 charge.
